Frequently Used Forms

| General | Registration Add, Drop, and Withdraw | This form is required if registering in person with an academic advisor. |
| Graduate | Change of Program Request Form | This form is used to change a program (major) and must be completed by degree-seeking students wishing to change their majors. Please note that the change of major form can not be used to add a second degree; a separate admissions application must be submitted. |
| Graduate | Transfer Pre-Approval Request | This form is used to pre-approve coursework at another university that will be used to satisfy graduation requirements. A number of restrictions apply; please review the instructions on the form. |
| Graduate | Recommendation Form | This form is to recommend students to a specific program. |
| Graduate | Transfer Credit Request Form | All transfer credit requests must be completed within the student’s first year of admission into the degree program. No transfer request forms will be accepted for review for non-degree seeking students. Please review the complete list of transfer request rules included in the form. |
| Graduate | Waiver Request Rules/Form | All waiver credit requests must be completed within the student’s first year of admission into the degree program. No waiver request forms will be accepted for review for non-degree seeking students. Additional information on waivers is outlined under Degree Plans. |
| Graduate | MAS 6102 Waiver Request | The MS professional development course, MAS 6102, was designed for JSOM graduate students to improve internship and workplace readiness and may be a required program prerequisite under a student’s degree requirements in their Catalog. Waivers will only be considered if the student meets the criteria outlined on the MAS 6102 waiver form. |
