This form is to register for an independent study with a specific full-time faculty sponsor.
Undergraduate Change of Major/Minor Request Form This form is used to change an undergraduate program (major or minor) and must be completed by degree-seeking students wishing to change their majors. Please note the change of major form can not be used to add a second degree; a separate admissions application must be submitted.
Graduate Change of Program Request Form This form is used to change a program (major) and must be completed by degree-seeking students wishing to change their majors. Please note the change of major form can not be used to add a second degree; a separate admissions application must be submitted.
Graduate Pre-Approval Request Form This form is used to pre-approve coursework at another university that will be used to satisfy graduation requirements. A number of restrictions apply; please review the instructions on the form.
Graduate Recommendation Form This form is to recommend students to a specific program.
Graduate Transfer Credit Request Form All transfer credit requests must be completed within the student’s first year of admission into the degree program. No transfer request forms will be accepted for review for non-degree seeking students. Additional information on transfer credit is outlined under Degree Plans.
